What is Glimepiride?

Generic Glimepiride is an oral antidiabetic drug. Second-generation sulfonylurea drugs cause less pancreatic exhaustion, reduce the risk of hypoglycemia, and Glimepiride is less likely to cause weight gain compared to other hypoglycemic drugs. Glimepiride rarely causes cardiovascular complications. The action of the drug starts quickly and is well absorbed. The elimination half-life of Glimepiride is 5-9 hours.

Glimepiride is prescribed for type 2 diabetes that is difficult to control with diet and physical exercises only, as monotherapy or in combination with Metformin, insulin or other oral or injectable antidiabetic drugs.

Dosage of Glimepiride

The dosage of Glimepiride is set individually based on regular monitoring of blood glucose concentrations.

If the patient has a hypoglycemic reaction when taking 1 mg of Glimepiride per day, this indicates that adequate control of blood glucose levels can only be achieved by following a proper diet low in easily digestible carbohydrates.

Blood and urine glucose concentrations should be measured regularly during treatment. In addition, it is recommended that the proportion of glycosylated hemoglobin be determined regularly.

If the patient forgot to take the next dose of the drug, it should not be increased at the next intake. If an accidental dose of Glimepiride is taken in excess of the recommended dose, it should be immediately reported to the physician.

Conditions that may also require adjustment of the dose of Glimepiride:

  •  decreased body weight in patients;

  •  changes in the patient's lifestyle (changes in diet, regular meals, amount of physical activity);

  • occurrence of other factors that lead to the development of hypoglycemia or hyperglycemia.

Treatment with Glimepiride 2 mg is usually carried out for a long time, only the doctor can cancel the drug.

Typically, the initial dose is 1 mg once a day. If necessary, the daily dose may be increased. When increasing the dose of the drug, it is necessary to consider the concentration of glucose in the blood and increase the dose gradually, at intervals of one to two weeks as follows: 1 mg - 2 mg - 3 mg - 4 mg - 6 mg.

Maximum dose of Glimepiride

The maximum daily dose of Glimepiride is 6 mg, and it is not recommended to exceed it. It is recommended to take Glimepiride before a large meal (frequently before breakfast). It is very important not to skip meals after taking the drug.

Contraindications of Glimepiride

Glimepiride is not prescribed for the following conditions:

  • stressful situations (severe trauma, burns, surgery, severe infections with fever, septicemia)

  • exhaustion, starvation, following a hypocaloric diet (less than 1000 kcal/day)

  • disorders of absorption of food and drugs in the gastrointestinal tract (with intestinal obstruction, intestinal paresis, diarrhea, vomiting)

  • chronic alcoholism, acute alcohol intoxication

  • galactose deficiency and intolerance, glucose-galactose malabsorption (because the drug contains lactose monohydrate)

  • diabetes of the first type

  • diabetic ketoacidosis, diabetic precoma and coma

  • leukopenia

  • severe abnormalities of the liver

  • severe renal dysfunction (especially if the patient is on hemodialysis)

  • hypersensitivity to Glimepiride or any additional component of the drug, to other sulfonylurea derivatives or to sulfonamides (risk of hypersensitivity reactions)

  • pregnancy and lactation

  • childhood under 18 years of age

Glimepiride reduces the concentration and speed of psychomotor reactions of the patient. This may adversely affect the ability to drive vehicles or operate various machines and mechanisms. Therefore, during treatment with this drug, it is better to refrain from driving a car.

Glimepiride and alcohol

Alcohol can cause a decrease in blood sugar levels not only immediately after consumption, but also 24 hours later, which can lead to dangerous side effects.

Taking Glimepiride and alcohol at the same time increases the risk of lacto acidosis, which causes lactic acid to build up in the blood and can lead to symptoms such as nausea and weakness.

Sulfonylurea drugs interact with alcohol and cause dizziness, nausea, and low blood glucose levels, which can lead to fainting and even death if not treated in time. It is important to stop drinking alcohol and seek medical attention.

Overdose

After ingestion of a large dose of Glimepiride, hypoglycemia may develop, lasting from 12 to 72 hours, which may recur after the initial recovery of blood glucose concentrations. In most cases, observation under hospital conditions is recommended. Increased sweating, anxiety, tachycardia, increased BP, palpitations, heart pain, arrhythmia, headache, dizziness, increased appetite, nausea, vomiting, apathy, sleepiness, restlessness, aggressiveness, concentration disorders, depression, confusion, tremors, paresis, sensitivity disorders, convulsions of central genesis may occur. Sometimes the clinical picture of hypoglycemia may resemble a stroke. Development of coma is possible.

Treatment: includes induction of vomiting, copious drinking with activated carbon (adsorbent) and sodium picosulfate (laxative). If a large amount of Glimepiride is taken, gastric lavage is indicated, followed by administration of sodium picosulfate and activated charcoal. As soon as possible begin administration of Dextrose, if necessary in the form of 50 ml of 40% solution by injection, followed by infusion of 10% solution, with careful monitoring of blood glucose concentration. Further treatment should be symptomatic.

Does Glimepiride cause hypoglycemia?

Glimepiride may cause a sharp decrease in blood glucose levels if used improperly and in excess of the dosage.

It can be difficult to distinguish between ordinary intoxication and hypoglycemia, because the symptoms of these conditions are similar. Usually, when there is a sharp drop in blood sugar levels, the diabetic is disturbed by:

  • tremors of the hands (begin to shake);

  • increased sweating;

  •  severe headaches;

  •  dizziness;

  • vomiting;

  • recurrent nausea;

  • confusion;

  • difficulties with orientation in space;

  • speech disorders (becoming slurred and incoherent).

If symptoms of precoma have been observed, the following pre-hospital care should be performed:

1.  Give the patient a sweet candy

2.  Feed food that is high in carbohydrates

3.  It is acceptable to give the patient a sweet tea or fruit drink

In case of loss of consciousness, more active actions are required to prevent the person's condition from worsening. A clear course of action should be followed, which includes the following principles of emergency care for hypoglycemic coma:

1. The unconscious person should be placed on his or her side.

2. If there are pieces of food (vomit) or other objects in the mouth, remove them.

3. Call an ambulance.

If the patient with diabetes mellitus often has a hypoglycemic state, it contributes to rapid vascular damage to the eyes (retinopathy).

It also frequently affects the cardiovascular system and the brain, which can lead to heart attack and stroke. That is why sudden hypoglycemia should not be allowed, and pre-hospital care should be provided in time.

The best time to take Glimepiride
