What is Ivermectin?

Ivermectin is a drug that is effective against many nematodes (roundworms). Therefore, the active ingredient is used orally or topically for the following indications:

  1. Gastrointestinal strongyloidiasis (anguillulosis, pygmy ascaridae)
  2. Suspected or diagnosed microfilaremia in patients with lymphatic filariasis
  3. Scabies (pruritus) caused by Sarcoptes scabei in people with a clear clinical or parasitological diagnosis
  4. Topical treatment of inflammatory lesions (papulopustular) rosacea
  5. The drug's active ingredient is FDA approved for the treatment of onchocerciasis (river blindness). Ivermectin tablets are also used to treat COVID-19.

Pharmacokinetics of the drug

The pharmacokinetics of the drug consists of sequential steps: absorption, distribution, metabolism, and excretion.

Absorption. Ivermectin is absorbed to a small extent after topical application. Absorption of Ivermectin pills is significantly higher, with maximum plasma levels reached after approximately four hours.

Distribution. More than 99% of the active substance is bound to plasma proteins, mainly serum albumin. Ivermectin passes poorly through the blood-brain barrier.

Metabolism. The drug is metabolized by the CYP3A4 enzyme. 

Excretion. The drug is excreted in the feces and less than 1% via the kidneys. Half-life after a single dose is 12 hours, while metabolites have a half-life of about three days. With regular topical administration, the half-life increases to six days.

How to take Ivermectin?

Ivermectin is taken orally as a tablet or applied to the skin as a hydrophilic cream.

Oral administration of Ivermectin is recommended on an empty stomach. Tablets are washed down with water. For children under six years of age, they can be crushed beforehand. The dosage depends on body weight and indications.
