Zestril: Uses
Zestril is a peptidyl dipeptidase inhibitor. It affects the inhibition of ACE, which is the main catalyst accelerating the conversion of angiotensin I into the vasoconstrictor peptide, angiotensin II, responsible for stimulating aldosterone secretion by the adrenal cortex.
Generic Zestril is an exact copy of the active drug Lisinopril. The generic is considered as safe and effective as the original drug.
Lisinopril reduces BP primarily by inhibiting the renin-angiotensin-aldosterone system (RAAS). At the same time, Lisinopril has an antihypertensive effect even in patients with low renin levels.
ACE is identical to kinase II, an enzyme that promotes bradykinin breakdown. Against the background of the drug action, there is a decrease in arterial systolic and diastolic pressure. In addition, Zestril tablets reduce total peripheral resistance in the renal vessels and improve blood circulation in the kidneys.
What does Zestril use for?
You can order Zestril online in our online pharmacy. Before use, read the instructions. The drug is prescribed by a doctor for various indications:
- arterial hypertension (in monotherapy or with other antihypertensive agents);
- chronic heart failure (as part of combined therapy to treat patients taking foxglove drugs or diuretics);
- early treatment of acute myocardial infarction (in the first 24 hours with stable hemodynamic parameters to maintain these parameters and prevent left ventricular dysfunction and heart failure);
- diabetic nephropathy (reducing albuminuria in insulin-dependent patients with normal BP and insulin-independent patients with arterial hypertension).
In most patients, the antihypertensive effect appeared 1-2 hours after oral administration, and the maximum effect is observed after 6-9 hours. Stabilization of the therapeutic effect was observed 3-4 weeks after the start of treatment.
Zestril: Dosage
To be taken orally, regardless of meals. The recommended starting dose of Zestril for patients not taking hypotensive agents is 10 mg once daily. If there is no therapeutic effect, the dose is increased every 2-3 days by 5 mg/day to an average therapeutic dose of 20-40 mg/day.
The usual maintenance dose is 20 mg once daily. The maximum daily dose of Zestril (Lisinopril) is 40 mg once daily (in clinical trials, the maximum dose of Lisinopril has been 80 mg/day, but increasing the dose above 40 mg/day usually does not lead to further BP reduction). Therapeutic effect develops 2-4 weeks after treatment start, which should be considered when increasing the dose. In case of insufficient therapeutic effect, it is possible to combine Generic Zestril with other hypotensive agents.
If the patient was previously treated with diuretics, their intake should be discontinued 2-3 days before the start of Zestril. If this is not possible, the initial dose of the drug should not exceed 5 mg/day. After the first dose, a physician should be observed for several hours because a marked decrease in blood pressure may occur.
Zestril: Overdose
When Zestril 20 mg is used improperly, an overdose occurs. Symptoms: arterial hypotension, vascular failure, electrolyte imbalance, renal failure, hyperventilation, tachycardia, increased heart rate, bradycardia, dizziness, restlessness, and cough.
Treatment: intravenous administration of the saline solution. In cases of arterial hypotension, the patient should be placed horizontally on his back and his legs raised. If possible, angiotensin II or catecholamine administration may be applied. If the drug has been used recently, measures should be taken to eliminate Zestril from the body (e.g., induce vomiting, gastric lavage, use of absorbents, and sodium sulfate). The use of a pacemaker is indicated if bradycardia is resistant to therapy. Vital signs, serum electrolytes, and creatinine concentrations should be checked frequently.
Contraindications
- Despite the effectiveness of the drug, Zestril tablets are prescribed by a doctor, since there are various contraindications:
- Hypersensitivity to the active ingredient and other excipients included in the drug, as well as generally undesirable reactions with ACE inhibitors.
- History of angioedema (including after use of ACE inhibitors, idiopathic and hereditary Quincke's edema).
- Aortic or mitral stenosis or hypertrophic cardiomyopathy with significant hemodynamic disorders.
- Bilateral stenosis of the renal artery or stenosis of the artery of the single kidney.
- Acute myocardial infarction with unstable hemodynamics.
- Cardiogenic shock.
- Patients with serum creatinine levels ≥220 μmol/L.
- Pregnant and lactating women.
Zestril tablets are contraindicated in patients with lactose intolerance, as the composition contains lactose.
Zestril: side effects
In most cases, patients tolerate this drug well. However, the instructions for use of Zestril warns that in some cases the following side effects may be noted:
- headache;
- dizziness;
- dyspeptic disorders;
- increased fatigue;
- excessive drop in blood pressure;
- impaired consciousness;
- somnolence;
- impaired sensitivity (paresthesias);
- chest pain;
- shortness of breath;
- dry cough;
- bronchospasm;
- tachycardia or bradycardia;
- changes in blood count (anemia, neutropenia);
- impaired sense of taste;
- abdominal pain;
- increased sweating;
- hair loss;
- erectile dysfunction;
- muscle pain;
- tremor;
- photosensitization;
- asthenia.
One of the most serious possible complications is the development of myocardial infarction in patients with CHD (if the required dose is exceeded and BP drops rapidly).
Zestril in pregnancy and breastfeeding
Zestril instructions for use are categorical: this cardio protector is not prescribed during pregnancy.
If you become pregnant, the course already started should be immediately interrupted! ACE inhibitors when taken by a pregnant patient in the second and third trimesters may cause fetal renal insufficiency and skull bone hypoplasia. There is a possibility of intrauterine death of the child.
During lactation, Lisinopril is also contraindicated (there is a possibility of penetration of the active ingredient into the mother's milk).

What you should know about brand and generic drug alternates.
Prescription drugs are a costly medical expense, especially for the elderly, those who are chronically ill and those with no or minimal drug coverage on their Health Insurance. However, each state in America has a law that allows pharmacists to substitute less expensive generic drugs for many brand name products. Depending on your prescription needs, your savings could be very significant. Before you talk with your doctor or pharmacist about switching, there are things you should know about generic drugs.
What is the difference between a generic and brand name drug?
Nothing except for the name, packaging, and the price, a generic drug is called by its chemical name; manufacturers assign their own brand name. The generic products have the same ingredients. Standard practice and most state laws require that a generic drug be generically equivalent to its brand name counterpart. That is, it must have the same active ingredients, strength, and dosage form, be it a pill, liquid, or injection. The generic drug also must be therapeutically equivalent. It must be the same drug chemically and have the same medical effect.
Do all drugs have generic equivalents?
No. Some drugs are protected by patents and are supplied by only one company. However, when the patent expires, other manufacturers can produce its generic version. Currently, about half the drugs on the market are available in generic form.
Will my doctor automatically prescribe generic drugs?
It depends on the physician. You can ask your doctor to write a prescription permitting substitution of a generic drug product when appropriate or to define the chemical name for the prescription. This will ensure you are purchasing the correct Mexico drug. Generic products will be as effective and less costly. Or, you can order only brand name products if you wish. www.MedsMex.com.com Mexico pharmacy offers a wide selection of both Brand Name & Generic Mexico medicine. Our Mexican generic drugs are of the highest quality, selected only from the most reputable manufacturers, and are safe to replace brand name medications costing much more.
